We are seeking a highly skilled Clinical Social Worker III to join our team at the Health and Human Services Commission. As a key member of our interdisciplinary team, you will provide complex social services work, including serving as a liaison between patients, legally authorized representatives, and other stakeholders to ensure ongoing care, treatment, and support.
Key Responsibilities- Assess individual support systems and service needs, including participation in interdisciplinary team meetings as needed.
- Design and provide individual and group trainings to promote the exercise of rights and patient transition.
- Assist with the coordination of furloughs, transfers, transitions, and discharges, as needed.
- Focus on complex cases and concentrate on post-discharge placement and support.
- Work under limited supervision, with considerable latitude for the use of initiative and independent judgment.
To be successful in this role, you will need to have a strong background in social work, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment. You will also need to be able to maintain confidentiality and handle sensitive information with discretion.
