Job Summary

We are seeking a skilled Aacute Care Home Nurse to join our team at UChicago Medicine Home Care. As a Hospital at Home Registered Nurse, you will provide high-quality, at-home acute care to various patient populations within our Acute Hospital Care at Home model.

Estimated Salary

$93,000 - $123,000 per year, depending on experience.

Job Description

Key Responsibilities:

  • Provide acute care services to patients in their homes, utilizing monitoring, assessment, planning, intervention, and critical thinking skills.
  • Navigate patients and families across the continuum of the UChicago Medicine Hospital at Home phases of care.
  • Monitor and respond to biometric data per UChicago Medicine Hospital at Home protocols.
  • Conduct home and audio/visual patient care visits as directed by the care team and/or protocol.
  • Ensure all interventions in the patient's care plan are implemented efficiently with a patient-centered approach.
  • Leverage resources and expertise of partners and supplier partners to implement collaborative work-flows, standards, policies, protocols, guidelines, and documentation systems to support safe, reliable, high-quality, evidence-based care.
  • Adhere to clinical protocols and obtain necessary training to provide care within the context of evidence-based care.
  • Promote professional practice and a culture of safety; engage in process improvement efforts.
  • Provide protocol-driven assessment, care coordination, and interventions as appropriate informed by UChicago Medicine Hospital at Home protocols for diverse patient populations.
  • Exhibit sound clinical decision-making and critical thinking skills.
  • Maintain and practice in accordance with nursing guidelines per state and hospital regulations.
  • Facilitate admissions to Hospital at Home utilizing UChicago Medicine Hospital at Home guidelines.
  • Utilize nursing knowledge to identify, prevent, and/or solve complex acute and/or recurring patient care problems.
  • Share expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise.
  • Support research efforts to advance knowledge and promote evidence-based practice.
  • Perform other duties and responsibilities as assigned by the Nurse Manager.
  • Evaluate incidents and participate in quality and safety investigations and root cause analyses.

Requirements

  • 2 years of Registered Nurse experience in an acute care setting, with home health experience preferred.
  • Valid driver's license, reliable vehicle, and proof of vehicle liability insurance.
  • Ability to communicate effectively in English.
  • Demonstration of core competencies completed within 90 days.
  • Completion of all position-specific unit-specific competencies within 12 months.
  • Associate Degree in Nursing; BSN degree or higher preferred.
  • Current CPR certification required by AHA.
