Job Summary

We are seeking an experienced Primary Care Physician to join our team as a Medical Director of Patient-Centered Care. This full-time role based in Littleton, CO, will be responsible for managing a patient panel with a broad array of needs, conducting a mix of acute, chronic, and well visits, treating patients in-office or in testing centers, and conducting occasional tele-health visits.

The ideal candidate will have at least 2 years of experience practicing in an outpatient primary care setting, be Board Certified in Internal or Family Medicine, and have a passion for human-centered primary care. They will also demonstrate effective communication skills, clinical proficiency in evidence-based primary care, and a desire to be part of a team dedicated to changing healthcare delivery.

Key Responsibilities
  • Manage a patient panel with a broad array of needs, including acute, chronic, and well visits
  • Treat patients in-office or in testing centers, and conduct occasional tele-health visits
  • Collaborate with in-office teammates and virtual clinical teams to ensure seamless care coordination
  • Supervise one or more NP or PA colleagues
Required Skills and Qualifications
  • Board Certification in Internal or Family Medicine (or Board Eligible with plans to obtain certification within 1 year)
  • At least 2 years of experience practicing in an outpatient primary care setting
  • Patient-centered care philosophy and commitment to delivering high-quality, evidence-based care
  • Effective communication and interpersonal skills
