Position Summary

The Physical Therapist (PT) is responsible for evaluating and assessing the needs of referred patients and formulating treatment plans in a home care environment.

The PT provides therapy services defined in treatment plans while working cooperatively with physicians and case managers.

The PT performs a variety of tasks and is encouraged to use a certain degree of creativity and latitude.

Qualifications
Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy, current and unencumbered licensed to practice as a Physical Therapist, and two years of experience as a Physical Therapist.
