Job Summary
We are seeking a skilled Patient Navigator II to join our team. This role will involve establishing contact with cancer patients, survivors, and caregivers to raise awareness about information resources, support programs, and services. The successful candidate will guide patients through the cancer care continuum, identifying barriers to treatment and assisting with access to needed resources.

Duties and Responsibilities
The key responsibilities of this role include:
  • Establishing contact with cancer patients, survivors, and caregivers to raise awareness about information resources, support programs, and services;
  • Guiding patients through the cancer care continuum, identifying barriers to treatment, and assisting with access to needed resources;
  • Connecting patients with community and social support services;
  • Facilitating interactions and communication with healthcare staff, providers, and resources;


Requirements and Qualifications
To be considered for this role, candidates must possess a High School Diploma/GED, Patient Navigator Certification, and a Medical Assistant Certificate. Relevant experience working in a healthcare setting and with oncology patients and families is preferred. Successful applicants will demonstrate strong knowledge of healthcare systems, medical terminology, and HIPAA regulations. They will also be proficient in navigating electronic medical records and other computer software systems.

Compensation and Benefits
The estimated annual salary for this position is $54,597 - $66,699, reflecting a competitive compensation package that includes benefits such as holidays, paid time off, retirement, medical, dental, vision, and life insurance. Additionally, eligible employees may participate in the Kern County Employees' Retirement Plan.
