About the Facility: Our long-term care facility offers post-acute rehabilitation services, long-term/skilled care, and outpatient therapy programs. As a Long-Term Care Specialist, you will work closely with our interdisciplinary team to provide high-quality care and support to our residents.

Responsibilities:

  • Provide resident-centered nursing care and daily living assistance to assigned residents under the supervision of a registered nurse (RN).
  • Assist with basic health care needs, including bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support.
  • Aid providers and nursing staff members with procedures, if needed.
  • Collect non-invasive body fluid specimens and gather vital signs.
  • Provide housekeeping and laundry services to assist residents with keeping living areas clean and operational.

Requirements:

  • High school graduate or general equivalency diploma (GED) preferred, but not required.
  • Minimum age of 16.
  • Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement.
  • CNA training and certification required and provided by facility.
  • Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver's license.
