Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at LewisGale Medical Center. As an LPN, you will play a vital role in delivering high-quality patient care and contributing to the well-being of our patients and their families.

Responsibilities
  • Deliver individualized, comprehensive care to patients, working collaboratively with the RN and other members of the care team.
  • Perform a variety of delegated basic patient care services, including medication administration, vital sign monitoring, and wound care.
  • Document patient information accurately and timely, using electronic medical records and other tools as needed.
  • Participate in multidisciplinary rounds and hourly rounds to ensure seamless communication and coordination of care.
  • Teach patients and families about health promotion, disease prevention, and self-care techniques.
  • Collaborate with other healthcare professionals to develop and implement individualized care plans.
Requirements
  • Graduate of a nursing program.
  • Current LPN licensure in the Commonwealth of Virginia or eligibility for licensure.
  • Basic Life Support (BLS) certification.
  • Two years of LPN experience preferred.
