Home Care LPN Job Opportunity

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Green Bay, Wisconsin. As a Home Care LPN, you will provide high-quality nursing care to patients in their homes, working evening and weekend shifts.

Job Summary

The Home Care LPN is responsible for delivering direct nursing care to patients, as prescribed by physicians, in their homes. This includes recording clinical and progress notes, coordinating patient care activities, and providing the highest quality of patient care in the assigned geographic area.

Responsibilities
  • Provides high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
  • Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
  • Actively participates in the agency's care coordination process, including timely reporting patients' needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
  • Ensures confidentiality of patient health information per agency policy.
Requirements
  • Must meet educational/experience requirements outlined in the laws of licensure states.
  • A minimum of one year of acute care experience preferred; previous home care experience preferred.
  • Must have current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification required.
