We are seeking a Patient Transition Expert to join our team. As a Patient Transition Expert, you will be responsible for ensuring that patients experience a seamless transition from hospital to home.

  • Salary: $85,000 - $110,000 per year
  • Benefits: Comprehensive health, dental, and disability benefits, 401(k) program with company match, generous paid time off

Responsibilities include delivering exceptional patient care, building strong relationships with referral sources, and collaborating with care teams to ensure smooth transitions.

To be successful in this role, you must have a proven track record of delivering high-quality patient care, excellent communication skills, and the ability to work effectively in a fast-paced environment.

The ideal candidate will have a RN/LVN/PTA license, valid state driver's license, and reliable transportation, as well as two years of clinical experience, one year of home health or healthcare sales experience preferred.
