Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Curana Health. As a Post-Acute Care Specialist, you will provide high-quality, patient-centered care to seniors in skilled nursing facilities, assisted living communities, and other senior living settings.

Key Responsibilities
  • Conduct comprehensive clinical evaluations and develop individualized care plans for residents.
  • Provide ongoing evaluation and treatment of chronic and co-morbid illnesses, as medically appropriate.
  • Coordinate care with collaborating physicians and other healthcare professionals to ensure seamless transitions and optimal patient outcomes.
  • Conduct Annual Wellness Visits and Annual History and Physicals as requested by facilities.
  • Assist with facility-wide illnesses and coordinate care with other disciplines as needed.
Requirements
  • Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
  • Masters Degree in Nursing or Physician Assistant Studies.
  • Strong leadership, interpersonal, and problem-solving skills.
  • Excellent written and oral communication skills.
