Responsibilities- Conduct comprehensive assessments of residents' health status and develop individualized care plans
- Administer medications and treatments according to prescribed protocols and monitor residents' vital signs and health condition
- Communicate effectively with residents, families, and interdisciplinary team members and coordinate care with physicians, therapists, and other healthcare providers
- Provide guidance and support to caregivers and other staff members and participate in care conferences to review resident care plans and progress
- For RN: Bachelor's or Associate degree in Nursing (BSN or ADN) from an accredited school of nursing and currently licensed in the State of Colorado
- For LPN: Completion of a state-approved practical nursing program and current LPN license in the State of Colorado
- Current CPR certification and experience in geriatric care or assisted living preferred
- Strong clinical assessment and critical thinking skills and excellent communication and interpersonal skills
