As a member of our experienced Case Management team, you will play a vital role in transition planning for our high-risk population. You will work on a consult basis, collaborating with a multidisciplinary team to provide patient advocacy and support. This is a great opportunity to develop your critical thinking skills and learn in a fast-paced environment.

Responsibilities
  • Initial assessment and development of transition plans
  • Coordination with a multidisciplinary team
  • Patient advocacy
  • Ability to delegate tasks to a Community Health Worker
