Under the direct oversight of the Program Supervisor and the overall guidance of the Program Manager, this role involves performing highly advanced (senior-level) social services and case management tasks.
The position entails providing comprehensive case management for various programs, including those for Children and Pregnant Women, Newborn Screening, Childhood Lead Poisoning Prevention, Personal Care Services, Children's Health Insurance, and Child Protective Services, as well as support for families in the Children with Special Health Care Needs Program.
Responsibilities include assisting families with eligibility determination processes, conducting home visits, completing intakes, Family Needs Assessments, Service Plans, and Follow-up Services for assigned families. The role also involves linking families to community resources and conducting thorough assessments to ascertain eligibility and necessity for Personal Care Services and Community First Choice.
Effective communication of program services and updates to consumers and the public is essential, as is active participation in Community Resource Coordination Groups, case management coalitions, and community organizations. Additionally, the position requires providing information and referral services, completing administrative tasks, special assignments, and monthly reports in a professional and timely manner.
This role operates with minimal supervision, allowing for significant initiative and independent judgment. Collaboration with public health staff during regional or state emergencies is also a key component, alongside maintaining confidentiality.

Client Services (45%):
Responsible for maintaining case files and providing specialized social work consultation and assessment for families of children with medical fragility and other identified groups to determine eligibility for Personal Care Services. Conducts Needs Assessments on all active case management cases and develops Individual Service Plans with follow-up assessments. Follow-up visits with families are necessary to evaluate service delivery and ongoing needs. Attendance at school meetings or group conferences for clients receiving case management may be required. Proper and timely documentation of all relevant information is essential.
Community & Outreach (20%):
Collaborates with community organizations, public health entities, and regional program staff. Participation in community events related to case management needs is expected. Provides presentations and information to stakeholders and community members as necessary. Engages with local, state, and federal social service entities to enhance services for families with children facing health risks and special health care needs. Attends training sessions to expand knowledge of community resources.
Reporting (15%):
Accountable for reporting all activities related to social health and support services to meet regional program goals. Ensures accurate documentation of case-related and community activities in the designated database as per central office or regional requirements. Maintains records of all work events within the appropriate time frames.
Quality Assurance (15%):
Develops corrective action plans following the results of quality assurance audits. Maintains documentation of QA audits and records all corrective actions. Assists in training new staff, volunteers, and social work interns, and participates in peer reviews for regional quality assurance.
Adherence to Policy & Other Duties (5%):
Actively participates in disaster response and recovery efforts or Continuity of Operations activation. Performs additional duties as assigned, including programmatic processes for upcoming events and conferences. Maintains an acceptable driving record and a valid driver's license. Keeps updated knowledge of Human Resources, Information Technology, Administrative Policy, and Program Policy. Timely reporting of time, labor, and travel information is required.

Awareness of cultural and socioeconomic issues related to health and disabilities.Proficiency in using computer software for word processing, spreadsheets, databases, electronic mail, and the Internet, particularly MS Word, PowerPoint, Excel, Access, and Outlook.
