A Home Health Aide (HHA) is a vital member of our healthcare team, providing essential care and support to patients in their homes or healthcare settings. As a Cardiac Home Health Aide, you will work under the supervision of a Registered Professional Nurse to deliver high-quality care and services to patients with complex health needs.
Responsibilities- Provide personal care and housekeeping services to patients, including assistance with daily living activities, bathing, dressing, and grooming.
- Assist patients with medication management, including administering medications and monitoring side effects.
- Support patients with mobility and transfer needs, including assistance with walking, transferring, and using assistive devices.
- Provide emotional support and companionship to patients, including listening to their concerns and providing reassurance.
- Collaborate with healthcare team members to develop and implement patient care plans, including setting goals and objectives.
- Document patient care activities and progress in accordance with agency policies and procedures.
- High school diploma or equivalent required.
- Successful completion of an approved Department of Health, Home Health Aide Training Program or Department of Health approved equivalency exam.
- Minimum 6 months experience as a Home Health Aide preferred.
- Good communication and interpersonal skills.
- Ability to read, write, and understand English.
- Valid driver's license and reliable transportation.
