Key Responsibilities:



  • Providing physical therapy services to geriatric and pediatric patients in their homes
  • Working closely with patients and families to understand their needs and goals
  • Developing and implementing individualized treatment plans to meet each patient's unique needs
  • Collaborating with other healthcare professionals to ensure comprehensive care
  • Maintaining accurate and timely documentation of patient progress and outcomes

Requirements:



  • Bachelor's or Doctorate degree in Physical Therapy
  • Physical Therapy License in NY State
  • Compassionate and comfortable working with geriatric and/or pediatric populations
  • Newly graduated physical therapists are welcome; we offer a VERY strong and structured mentorship program
  • Able to travel between the homes of patients in geriatric and pediatric populations
  • Timely submission of documentation
  • Eagerness to progress patients to their highest potential
