Position Overview:

As a Rehabilitation Therapy Assistant at Lexington Premier Nursing & Rehab, you will play a vital role in delivering therapeutic and wellness services to various senior living communities, including Assisted Living, Independent Living, and Memory Care.

Key Responsibilities:

Your responsibilities will include, but are not limited to:

  • Delivering clinically appropriate therapeutic interventions that are evidence-based and tailored to meet the individual needs of patients, as directed by a licensed physical therapist.
  • Identifying and facilitating referrals to other healthcare professionals to address the comprehensive needs of patients.
  • Documenting treatment progress and providing regular feedback to the physical therapist, patients, and their families regarding therapy outcomes and discharge planning.
  • Assisting in the training and orientation of staff and students in rehabilitation therapy practices.
  • Maintaining professional communication and collaboration with physicians, care staff, and interdisciplinary teams to ensure optimal patient care.
  • Completing documentation and billing in a timely and accurate manner, adhering to state regulations and company policies.
  • Ensuring patient safety and minimizing the risk of healthcare errors by managing equipment safely and maintaining a clean work environment.
  • Engaging in continuous professional development to enhance expertise in physical therapy practices.
  • Contributing to departmental growth by reviewing best practices and providing educational training in compliance with professional standards.
  • Participating in safety programs, patient conferences, and departmental training sessions as required.
  • Meeting productivity expectations set by the organization.
  • Treating patients and their families with dignity and respect, addressing their psychosocial and cultural needs.
  • Adhering to HIPAA and PHI guidelines to protect patient confidentiality.
  • Offering assistance to colleagues and participating in team functions and special projects as needed.

Qualifications:

To be successful in this role, candidates should possess the following:

  • Graduation from an accredited Physical Therapist Assistant program with relevant experience in rehabilitation within senior care settings preferred.
  • Current licensure as a Physical Therapist Assistant in good standing or eligibility for temporary licensure under supervision.
  • Basic Life Support/CPR certification.
  • Familiarity with Medicare payment systems and electronic health record systems.
  • Proficiency in physical therapy modalities and exceptional communication skills.
  • A passion for working with older adults and a commitment to providing high-quality care.
  • Strong motivation and initiative to achieve results and overcome challenges.
  • Professional demeanor with conflict resolution skills.
  • Commitment to professionalism and customer service in all interactions.

Physical Requirements:

This position requires the ability to lift and move up to 50 lbs, maintain a stationary position, and perform various physical tasks. Candidates should be able to communicate effectively and interact with team members throughout the organization.
