Job Description

This full-time position involves working independently in patients' homes, implementing prescribed plans of care, conducting assessments, and coordinating care. You will also be responsible for administering medication, treatments, and therapies, as well as documenting care provided. Strong communication skills and ability to follow up on client problems and concerns are essential.
