About the Role

Fairview Range is seeking a skilled Physical Therapist to join our Hibbing Home Care and Hospice team. As a dedicated member of our care team, you will play a vital role in delivering high-quality physical therapy services to our patients.

Key Responsibilities
  • Develop and implement comprehensive care plans tailored to the unique needs of clients across all age groups.
  • Collaborate with the care team to ensure seamless delivery of services, including education and support for clients and caregivers.
  • Maintain accurate and timely clinical records, adhering to agency policies.
  • Supervise Physical Therapist Assistants (PTAs) and contribute to the growth and development of the team.
Requirements
  • Current licensure as a Physical Therapist in Minnesota.
  • At least one year of experience in acute care, long-term care, or clinic settings.
  • Preferred qualifications include a Doctorate of Physical Therapy and experience in home care.
