About the Role:

At Sunrise Senior Living, we are committed to providing the highest quality of care and services to our residents. As a Care Manager, you will play a vital role in delivering exceptional care and support to our residents and their families.

Responsibilities:

  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Develop and implement Individualized Service Plans (ISPs) to meet the unique needs and preferences of each resident.
  • Collaborate with the community team to ensure compliance with OSHA requirements and promote a safe and healthy environment for residents and staff.
  • Respond to the dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
  • Participate in the development of ISP monthly updates and notify management of changes in resident condition.

Qualifications:

  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma/GED accepted and may be required per state regulations.
  • Previous experience working with seniors preferred.
