Job Title: Delaware Home Health Physical Therapist Opportunities

About the Job:
We are seeking a skilled and dedicated Physical Therapist to join our team in Lewes, Delaware. As a Physical Therapist at LHC Group, you will play a vital role in providing high-quality care to patients in their homes.

Responsibilities:
- Evaluate patient functional status and physical therapy needs
- Assist physicians in developing therapy plans of care
- Treat patients to relieve pain, develop, and restore function, and/or maintain maximum functioning using acceptable standards of practice
- Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals

Requirements:
- Current Physical Therapy licensure in the state of Delaware
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle

What We Offer:
- Competitive estimated salary range: $83,000 - $110,000 per year, based on experience and location
