We are seeking a highly skilled Physical Therapist to join our team at Advocate Aurora Health. As a Physical Therapist, you will be responsible for evaluating and treating patients with a variety of conditions, including musculoskeletal, cardiopulmonary, and neuromuscular disorders.
Key Responsibilities- Evaluate patients to determine the nature and extent of their condition, and develop a plan of care to address their needs.
- Implement individualized treatment plans, including exercises, modalities, and other interventions, to help patients achieve their goals.
- Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in performance improvement activities to identify and resolve problems affecting patient care outcomes.
- Master's degree in Physical Therapy from an accredited program.
- Current licensure as a Physical Therapist in the state of Wisconsin or Illinois.
- Basic Life Support (BLS) certification from the American Heart Association.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
