Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Full Time. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with healthcare teams to develop and implement personalized care plans.
Key Responsibilities:
- Develop and implement individualized care plans in accordance with physician orders and client input.
- Educate clients and their family members on specific needs and provide ongoing support.
- Orient and train primary caregivers to ensure optimal functioning levels for each client.
- Coordinate continuum of hands-on client care through documentation and timely communication with physicians and caregivers.
- Provide care in patients' homes using a variety of skills, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
Requirements:
- Active RN License in the state of application.
- Valid CPR certification.
- Preferred: Medicare Skilled Nursing experience and basic understanding of Oasis.
