Job Title: Critical Care LPN

BAYADA Home Health Care is seeking a skilled and compassionate LPN to join our HICU Health Services team in the Philadelphia, PA area. As a Critical Care LPN, you will have the opportunity to work in a team-based environment, delivering high-quality skilled nursing care to patients in transition from hospital to home.

Responsibilities:
  • Deliver quality skilled nursing care to patients in their homes
  • Work collaboratively with a team of healthcare professionals to provide comprehensive care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Provide education and training to patients and their families on self-care and disease management
  • Conduct waived testing, such as glucometer and PT/INR, as applicable
  • Provide tracheostomy and ventilator care, as applicable
  • Participate in ongoing education and training to stay current with best practices and industry developments
Requirements:
  • Current LPN license in the state of Pennsylvania
  • At least one year of verifiable work experience as an LPN
  • NICU, PICU, ICU, or home care trach/vent experience required
  • Demonstrated competence in performance of waived testing, tracheostomy/ventilator care, and infusion experience
  • Ability to work in a teaching setting to prepare and educate families and BAYADA Care team for the transition from hospital to home
