Position Overview
The Patient Care Manager plays a crucial role in overseeing the continuum of care for patients, ensuring that all services rendered are tailored to meet individual patient needs while adhering to high standards of quality and efficiency.
Qualifications
Applicants should possess a Bachelor's or Master's Degree in Social Work, with a preference for those holding an MSW. Limited licensure may be considered. A minimum of three years of recent experience in patient care is required, along with a comprehensive understanding of hospital operations.
Essential skills include strong interviewing, assessment, organizational, and problem-solving abilities, as well as the capacity to work independently. Candidates must demonstrate effective time management and prioritization skills.
Knowledge of the Case Management Society of America's standards of practice is essential. Candidates should be eligible to take the certification exam for either Certified Case Manager (CCM) or Accredited Case Manager (ACM) within two years of employment.
Key Responsibilities

The Patient Care Manager is expected to foster positive relationships with patients, families, and healthcare professionals, demonstrating excellent communication and negotiation skills throughout.
