Job Title: Home Health Physical Therapist

LHC Group is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Berwick, Pennsylvania.

About the Role:
  • As a Home Health Physical Therapist, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and improving quality of life.
  • You will work collaboratively with our interdisciplinary team to develop and implement personalized care plans, addressing the unique needs and goals of each patient.
  • With a focus on evidence-based practice and patient education, you will empower patients and their families to take an active role in their care and recovery.
Responsibilities:
  • Conduct comprehensive assessments of patients' physical function and mobility, identifying areas for improvement and developing targeted interventions.
  • Design and implement individualized treatment plans, incorporating a range of modalities and techniques to address patients' physical, emotional, and social needs.
  • Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families, promoting self-management and independence in daily activities.
Requirements:
  • Current licensure as a Physical Therapist in the state of Pennsylvania.
  • Graduate degree in Physical Therapy from an accredited program.
  • Minimum 1 year of experience in home health or a related setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
