How You Can Contribute

As a Licensed Practical Nurse LPN/LVN Care Provider, you'll be instrumental in helping us achieve our mission and values. Your key responsibilities will include:

  • Providing individualized care to patients in their homes, addressing their unique needs and preferences
  • Communicating effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Maintaining accurate records, documenting patient interactions, and reporting any changes or concerns to the supervising RN or physician
  • Collaborating with interdisciplinary teams to address patient needs and develop effective care strategies

Requirements:

  • Valid LPN/LVN licensure, current CPR certification, and a minimum of six months of hands-on nursing experience
  • Strong communication, problem-solving, and organizational skills
  • Flexibility to work varied schedules, including evenings, weekends, and holidays
  • Reliable transportation and ability to navigate diverse neighborhoods and communities
