Position Overview:
The Hospice Registered Nurse (RN) utilizes a Case Management approach to provide comprehensive nursing evaluations, planning, and care focused on enhancing the comfort and quality of life for patients and their families, in accordance with the interdisciplinary care plan and regulatory guidelines.

Key Responsibilities:
In this position, you will act as the main point of contact between the physician, patient/family, and hospice team. Your responsibilities will encompass:
  • Providing direct nursing care to hospice patients, managing a reasonable caseload, while offering support to the patient's family.
  • Leading an interdisciplinary team that includes LPNs, CNAs, and support personnel to improve the quality of hospice services.
  • Evaluating patient/family requirements and assisting in the creation and execution of personalized care plans.
  • Keeping precise clinical documentation for all assigned patients.
  • Engaging in quality assessment and performance enhancement initiatives within the organization.
Qualifications:
  • Current RN (Registered Nurse) license in Georgia.
  • Valid CPR certification.
  • Previous experience in hospice or home health environments.
  • Availability for on-call duties as necessary.
  • Access to a reliable vehicle, valid driver's license, and the minimum state-required liability auto insurance.
