We are seeking a highly skilled Clinical Pharmacist to join our team at Ochsner LSU Health System. As a Clinical Pharmacist, you will play a critical role in the medication management of assigned patients, ensuring their safety and well-being.
Key Responsibilities- Monitor and evaluate medication therapy for appropriateness, taking into account patient health status, institutional criteria, and cost-efficient measures.
- Provide education to healthcare providers, patients, and staff on medication management and patient care.
- Develop and implement pathways, best practices, and guidelines for medication therapy.
- Collaborate with healthcare teams to ensure seamless transitions of care.
- Stay up-to-date on current federal, state, and local laws, accreditation standards, and regulatory agency requirements.
- Current Pharmacy license in the state of practice.
- Completion of a PGY1 Pharmacy Practice Residency (or 3 years of related specialty experience).
- Strong leadership and teaching skills.
- Effective verbal and written communication skills.
- Ability to work in a fast-paced environment and adapt to changing situations.
