The RN Case Manager plays a vital role in coordinating care and services for selected patient populations throughout the acute care episode and across the continuum.
As a key member of the interdisciplinary team, the RN Case Manager assumes a leadership role in driving desired clinical and financial outcomes through effective utilization and monitoring of healthcare resources.
Responsibilities- Coordinates care and services for selected patient populations
- Works collaboratively with interdisciplinary teams to improve patient care
- Assumes a leadership role in driving desired clinical and financial outcomes
- Utilizes standards, guidelines, and protocols for care delivery
- Continuously improves care and outcomes through data analysis
- Minimum of 3 years of clinical nursing experience
- 5 years of clinical nursing and case management experience preferred
- Experience in case management field, including transitional planning, outcomes management, and assessment
- Professional knowledge of nursing theory and practice
- Accredited case manager certification (ACM-RN) through the American Case Management Association (ACMA) preferred
