Responsibilities for this Physical Therapist opportunity include:



  • Providing Physical Therapy services to patients under the appropriate guidelines established by each individual client facility
  • Conducting screening of patients in accordance with the policies of the client facility to determine need for intervention/treatment
  • Evaluating patients to obtain data necessary for treatment planning and implementation
  • Interpreting and communicating evaluation findings to patients, family, and other team members
  • Establishing individual patient schedules in conjunction with other team members
  • Developing treatment plans by identifying a problem list, including short and long-term goals and methods to achieve identified goals
  • Coordinates treatment plan with patient, family, and other team members
  • Implements and conducts treatment as outlined in treatment plan
  • Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders
  • Develops appropriate home or community programming to maintain and enhance the performance of the patient in his or her own environment
  • Documents results of patient's assessment, treatment, follow-up, and termination of services
  • Adapting treatment to meet the individual needs of the patient
  • Provides/recommends/constructs adaptive devices, orthotics or prosthetics
  • Trains patient, family and appropriate staff in the use or application of the above
  • Participates in discharge planning when applicable
  • Supervises and directs Physical Therapy Assistants in the implementation of the treatment plan, when applicable
  • Supervises students and volunteers, when applicable
  • Supervises Physical Therapy Aides and Rehabilitation Aides, when applicable
