Job Title: Family Nurse Practitioner

We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at Paragon Healthcare, a proud member of the Elevance Health family of companies.

Job Summary:

The Family Nurse Practitioner will be responsible for providing high-quality patient care within their scope of practice in the clinical setting. This includes administering ordered therapies, completing admission processes, performing physical assessments, and documenting all pertinent data in the patient's medical record.

Key Responsibilities:
  • Administer ordered therapies according to prescriber order, facility protocols, INS standards, and company policy and procedures.
  • Complete admission process as directed by Center Director.
  • Perform physical assessment on each patient as warranted and for new consults as warranted by the patient diagnosis.
  • Refer to the NP on duty for complete physical assessment as needed.
  • Document all pertinent data in the patient's medical record.
  • Report all significant changes or observations to the Center Director and be responsible for follow-up on any problem identified.
  • Communicate effectively with other members of the IV team on patient status and observations.
  • Maintain patient confidentiality at all times by abiding by HIPAA laws and regulations.
  • Evaluate the patient's response to therapy and document this finding in the medical record.
  • Educate patients and/or family members regarding therapy plan.
  • Participate in the Quality Improvement program.
  • Ensure that all medication orders are complete, appropriate, accurate, and up-to-date prior to treatment.
  • Participate in On-Call program, which may include after-hours, weekend, and holiday calls and visitations.
  • Provide clinical coverage for nurse practitioners on PTO/LOA and may also support new market launches.
  • May travel to worksite and other locations as necessary.
Requirements:
  • Requires an MS in Nursing and a minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
  • Experience with IVs required.
  • Current, active, and valid unrestricted NP license to practice as a healthcare professional with scope of license in applicable state required in applicable state required.
  • Requires active, current, and valid Family Nurse Practitioner Certification.
  • Multi-state licensure is required if this individual is providing services in multiple states.
  • Satisfactory completion of a Tuberculosis test is a requirement for this position and Hepatitis B vaccine or signed waiver.
Preferred Skills, Capabilities, and Experiences:
  • Healthcare experience with IV's strongly preferred.
  • 2+ years of experience with IV preferred.
  • Port, PICC & Peripheral Line experienced preferred.
  • Medication Mixing experience preferred.
  • Titration experience preferred.
Work Environment:

Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
