Job Overview

The LPN Nurse Consultant plays a crucial role in delivering Wellness Coordination services as outlined by relevant state regulations. This position involves overseeing employee training in essential health practices and managing wellness initiatives for program participants.

Key Responsibilities

A. Employee Training

  1. Facilitate training sessions on wellness protocols for new staff during orientation.
  2. Organize in-service training tailored to specific health monitoring needs.
  3. Conduct CPR and First Aid training for both new hires and current employees.
  4. Collaborate with the HR department to arrange necessary health screenings for new employees.

B. Wellness Coordination

  1. Deliver wellness coordination services in line with Individual Support Plans.
  2. Complete the State's Risk Mitigation Tool for assigned participants.
  3. Develop and uphold wellness coordination plans for individuals.
  4. Maintain high-risk management plans.
  5. Consult with interdisciplinary teams to assess health needs and update wellness strategies.
  6. Act as a liaison among individuals served, staff, and healthcare providers.
  7. Conduct face-to-face visits as per wellness plans and document interactions accordingly.

C. Quality Assurance

  1. Analyze medication error reports to identify training needs and recommend improvements.
  2. Provide re-training on medication administration for staff with identified errors.
  3. Verify medication records and physician orders during routine checks.
  4. Monitor changes in medication and healthcare directives.
  5. Coordinate the delivery of medications from pharmacies.
  6. Resolve medication-related issues and assist with PRN medication coordination.
  7. Work closely with the Quality Assurance Coordinator to ensure compliance with medication policies.
  8. Conduct audits of wellness coordination plans for all designated consumers.
  9. Prepare reports on medication errors and wellness audits, including suggestions for enhancements.
  10. Serve as a resource for staff regarding medication administration and wellness coordination concerns.

D. Team Collaboration

  1. Embrace and promote the organizational mission and values.
  2. Ensure confidentiality of consumer and personnel information.
  3. Perform additional duties as assigned.
Qualifications
  1. Current Indiana nursing license as an LPN is required.
  2. Preferred experience working with individuals with disabilities.
  3. Previous supervisory or training experience is advantageous.
  4. Strong verbal and written communication skills are essential.
  5. Ability to effectively present information and prepare concise reports.
  6. Certification from the Red Cross to teach CPR/First Aid is mandatory.
  7. Dependable transportation and valid Indiana driver's license are necessary.
  8. Physical ability to lift up to 35 pounds and travel between locations.
  9. Willingness to work flexible hours as needed.
