About the Role

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team. As a key member of our team, you will play a vital role in providing high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Conduct comprehensive in-home assessments, including medical history reviews, medication reconciliation, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses and develop care plans to address patient needs.
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination.
  • Educate patients on disease management, medication adherence, and healthy lifestyle choices.
Requirements
  • Active NP or PA license, with the ability to obtain national certification in the state of assignment.
  • Unencumbered licensure, free of any disciplinary actions or restrictions.
  • Ability to complete physical demands of the job, including lifting, navigating stairs, and sitting/standing for extended periods.
  • Reliable transportation for home visits.
Preferred Qualifications
  • 1+ years of clinical experience in a relevant setting.
  • Additional licensure in other geographic areas may be requested.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology is a plus.
  • Home health care or home visit experience is highly valued.
