Position Overview:
The primary role of the Patient Care Assistant is to provide essential daily nursing support in alignment with each resident's personalized care plan, adhering to all applicable federal, state, and local regulations.
Key Responsibilities:
1. Respond to resident call lights to assess and fulfill their needs.
2. Assist residents with daily living activities to promote their independence.
3. Ensure that all care is delivered with the utmost respect for privacy and dignity.
4. Help residents feel comfortable by providing assistance with tasks such as getting in and out of bed and offering hydration.
5. Support residents during meal times, including food preparation and feeding assistance as required.
6. Accurately document residents' intake and output.
7. Safely lift and transfer residents between their wheelchair, bed, and chair.
8. Measure and record vital signs and weights according to established protocols.
9. Transport residents to dining areas and activities, ensuring their safe return to their rooms.
10. Promptly report any changes in residents' conditions to the Nurse Supervisor.
11. Adhere to HIPAA regulations and maintain confidentiality.
12. Follow infection control procedures diligently.
13. Utilize proper body mechanics to ensure safety.
14. Provide companionship and high-quality care to all patients.
15. Maintain a clean and healthy living environment for residents.
16. Prioritize resident activities based on their current care requirements.
17. Exhibit punctuality and professionalism in all interactions.
18. Participate in the care planning process and implement care strategies as outlined.
19. Perform additional duties as assigned.
Qualifications:
- Previous experience as a Certified Nursing Assistant is preferred.
- Experience in skilled nursing settings for at least one year is preferred, but we welcome new graduates.
Licenses/Certifications:
- Current CNA Certification is required.
- CPR Certification is mandatory.
- Familiarity with Point Click Care (POC) software is highly desirable.
