About the Role
The ideal candidate will have at least one year of clinical experience as a Licensed Practical Nurse (LPN) and be licensed to practice in the State of Michigan. You will provide home-based care to patients with illnesses, injuries, and chronic diseases, working closely with a team of physicians, RNs, CNAs, and aides to deliver quality patient care.
Your Responsibilities
- Provide skilled nursing care to patients in their homes, including taking vital signs, conducting assessments, and documenting progress
- Collaborate with the interdisciplinary team to develop and implement patient care plans
- Assist patients with self-care techniques and educate family members on patient care and safety
- Work collaboratively with the healthcare team to coordinate patient care and plan for discharge
