Job SummaryThe Hospital Social Worker will provide patients and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses within a hospital setting. This role involves delivering professional and thorough social work services, including psychosocial assessment and intervention planning.
Key Responsibilities- Support patients and family caregivers through patient education and counseling
- Make referrals for other services as needed
- Work effectively with various internal and external partners, patients, families, and physicians to facilitate patient care
- MSW from a program accredited by the Council on Social Work Education (CSWE) or a similar accreditation body designated by the Minnesota Board of Social Work
- LGSW, LISW, or LICSW licensure - MN Board of Social Work
- 1 year of experience in healthcare or social services (2 years of social work experience in LICSW)
