Chronic Disease Manager

We are seeking a detail-oriented RN Care Manager to manage a panel of rising and high-intensity members. The ideal candidate will have strong communication and technical skills, with the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Engagement: Receives members from engagement and care teams, describes program expectations and goals, and completes self-efficacy and relevant condition-specific screeners.
- Assess/Intake: Conducts in-person clinical exams, triages members with behavioral health programming needs, and collaborates with care teams to determine member placement in different programs.
- Case Review and Care Planning: Develops care plans with members, collaborating with Community Health Partners on social needs as needed, and leads case conferences.
- Clinical Visits and Follow-Up: Performs regular check-ins, follows up on therapeutic interventions, and addresses member needs in a timely manner.
$85,000 - $110,000 per year
Requirements:- Graduate of an accredited school of nursing (R.N.)
- 3+ Years of experience
- Basic Life Support (BLS) certification
- Active RN License in the state(s) practicing
- Comfortable using technology to support members without in-person contact
- Understanding of scheduling platforms and electronic health record systems
- Strong problem-solving skills and adaptability to change
