Key Responsibilities- Provide direct nursing care to acutely ill and chronically ill patients in their homes, as prescribed by physicians.
- Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
- Work collaboratively with the healthcare team to develop and implement individualized care plans.
- Ensure confidentiality of patient health information and maintain accurate records.
- Current LPN licensure in the state of practice.
- Minimum one year of acute care experience, with previous home care experience preferred.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR certification required.
- Flexible schedules with weekly pay.
- One-on-one patient care.
