PennState Health is seeking a skilled Registered Nurse Navigator to join our team in Marysville, Pennsylvania.
Job Summary:The Registered Nurse Navigator plays a vital role in facilitating the patient journey, ensuring seamless navigation throughout the healthcare system upon discharge. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with various stakeholders.
Key Responsibilities:- Provide clinical guidance and support to scheduling coordinators and other staff members.
- Identify and recommend alternative locations of care when necessary.
- Assist in coordinating care transitions and ensuring a smooth patient experience.
- Collaborate with healthcare providers to develop individualized care plans.
- Monitor patient progress and adjust care plans as needed.
- Current licensure as a Registered Nurse in the state of employment or a multistate RN license through the interstate Nurse Licensure Compact.
- AHA BLS certification prior to the end of the orientation period.
