Description

We are seeking a motivated and passionate Physical Therapist to join our team in Home Care. As a Physical Therapist, you will be responsible for treating patients in their homes and developing individually designed programs to restore overall function and independence.

Key Responsibilities
  • Plan, prepare, and carry out treatment plans for patients in their homes
  • Develop a plan of care and goals in consultation with the physician, family, and patient
  • Timely and accurate documentation of visits
  • Effective communication with family members and other team members
Requirements
  • Physical Therapist Program Graduate
  • NY Physical Therapist License or eligible
  • Evaluates a patient's need for physical therapy and develops an appropriate treatment plan
  • Provide appropriate care and treatment to patients based on physician care plan, agency policy, and professional standards of care
