The LHC Group is seeking a skilled Home Health Physical Therapist to join our team in Roseland, New Jersey. As a key member of our interdisciplinary healthcare team, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Your expertise will help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate their progress.
About the Role
You will work closely with the physician to develop the therapy plan of care, observing and reporting on the patient's response to treatment and any changes in their condition. You will also treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
Key Responsibilities:
- Evaluate patients' functional status and physical therapy needs.
- Assist the physician in developing the therapy plan of care.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning.
- Instruct and inform participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
To be successful in this role, you must have:
- Current Physical Therapy licensure in the state of New Jersey.
- Current CPR certification.
- A current driver's license and vehicle insurance, as well as access to a dependable vehicle.
