Position Overview
The Healthcare Liaison Nurse plays a crucial role in fostering business development initiatives aimed at enhancing the facility's reputation as the leading post-acute care provider within the community, while adhering to all relevant laws, regulations, and standards set forth by Life Care.

Qualifications
• An associate's or bachelor's degree in nursing from an accredited institution.
• Active and current nursing license in the applicable state.
• Maintenance of a valid Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing throughout employment.
• Minimum of two (2) years of nursing experience.
• Previous experience in clinical systems or a post-acute care environment is preferred.

Key Competencies
• Willingness and ability to travel as needed.
• Exceptional verbal and written communication skills.
• Valid driver's license with a satisfactory driving record in accordance with Life Care standards.
• Advanced knowledge in nursing practices and the ability to make independent decisions when necessary.
• Familiarity with laws, regulations, and guidelines governing post-acute care facilities.

Core Responsibilities
• Conduct pre-admission assessments for patients at acute care hospitals or other referral sources.
• Navigate the managed care process effectively.
• Build and maintain productive relationships with healthcare providers.
• Collaborate with physicians to clarify orders and ensure comprehensive patient care.
• Deliver outstanding customer service and maintain a positive demeanor towards patients.
• Assist in patient evacuation procedures when required.
• Demonstrate consistent attendance and punctuality.
• Utilize critical thinking and sound judgment in daily operations.
• Work collaboratively within an interdisciplinary team.

Physical Requirements
• Ability to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently throughout the workday.
• Proficient in reading, writing, speaking, and understanding the English language.
