Job Summary:

The Licensed Practical Nurse (LPN) is a critical member of our team at Stony Brook Medicine, providing high-quality patient care and support to our patients and their families. As an LPN, you will work closely with our interdisciplinary team to deliver comprehensive care to our patients, including assessing their needs, developing and implementing care plans, and providing education and support to patients and their families.

Responsibilities:

• Assess patients' physical and emotional needs, and develop and implement individualized care plans to meet those needs.
• Provide direct patient care, including administering medications, performing treatments, and providing wound care.
• Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between departments.
• Educate patients and their families about their conditions, treatment options, and self-care techniques.
• Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) in the State of New York.
• One year of experience as an LPN in a hospital or healthcare setting.
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.

Working Conditions:

This position requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The LPN must be able to lift, push, and pull patients and equipment, and must be able to stand for long periods of time. The LPN must also be able to work in a variety of environments, including patient rooms, treatment areas, and administrative offices.
