About the RoleWe are seeking a skilled and compassionate LPN to join our team of healthcare professionals. As an LPN on our team, you will be responsible for administering quality, skilled nursing care to patients in their homes. You will work in accordance with physician orders and the Plan of Care, under the direction and supervision of a Registered Nurse and/or Director, Professional Services.
Responsibilities- Administer skilled nursing care to patients in their homes, in accordance with physician orders and the Plan of Care.
- Work collaboratively with other members of the healthcare team to ensure the provision of quality patient care.
- Provide patient education and support to patients and their families, as needed.
- Document patient care and progress in a timely and accurate manner.
- Participate in ongoing assessment and evaluation of patient care, identifying areas for improvement and implementing changes as needed.
- Graduate of an accredited LPN/LVN program.
- Minimum of one year experience as an LPN/LVN in a clinical care setting.
- Home care experience preferred.
- Current BCLS certification prior to delivering patient care.
