Job Overview

We are looking for a licensed Physical Therapist with at least one year of experience in home health care to provide personalized therapy sessions to adult and geriatric clients in their homes. As a member of our interdisciplinary team, you will work closely with other healthcare professionals to develop and implement comprehensive care plans that cater to the unique needs of each client.

About the Role
  • Make home visits to clients in designated geographic territories
  • Perform diagnostic tests and measurements, including mobility assessments, transfer status evaluations, and strength and endurance evaluations
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions
  • Educate and instruct clients and family members on rehabilitative care and activities necessary for independent living
Requirements and Qualifications

To be considered for this role, you must have:

  • A current PA Physical Therapist license
  • A minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills

Salary Estimate

The estimated salary for this role is $65,000 - $85,000 per year, based on industry standards and location.
