  • Perform skilled nursing visits as needed, delegating and coordinating care with LVN and HHA team members.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Current RN licensure in the state of Texas.
  • Minimum 3 years of experience in home health care or a related field.
  • Strong clinical skills and ability to work independently in a fast-paced environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to relocate to the Garland, TX area prior to starting work.
