Job Overview

We are seeking a dedicated Patient Care Coordinator to join our team at Riverside Health System. In this role, you will provide exceptional patient care and support to our patients and their families.

About the Role

  • You will be responsible for coordinating patient care activities, including scheduling appointments, communicating with patients and families, and ensuring timely and safe patient care.
  • You will work under the supervision of the Nurse manager and the Director and collaborate with other healthcare professionals to ensure optimal patient outcomes.
  • You will utilize communication, critical thinking, problem-solving, and decision-making skills to resolve patient concerns and address issues in a timely manner.
  • You will supervise and delegate tasks to non-professional personnel as needed to ensure efficient patient care.

Key Responsibilities

  • Develop and implement plans of care, treatments, and procedures that contribute to patient stability and outcomes.
  • Provide health counseling and education to patients and families on medications, procedures, and testing.
  • Communicate effectively with providers, vendors, and other team members to ensure timely and safe patient care.

Qualifications

  • Active RN license in the state of Virginia (or eligibility)
  • Bachelor's degree in Nursing or related field (desired)
  • 1-3 years of experience in a doctor's office setting (preferred)
  • CPR/BLS certification from American Heart Association/American Red Cross/American Safety and Health Institute
