Job Summary:

We are seeking a skilled RN Triage professional to join our team at Hospice & Palliative Care of Greensboro. This part-time remote position involves working every Saturday and Sunday, 7:00am - 4:30pm, supporting our Burlington and Greensboro campuses.

Key Responsibilities:
  • Triage calls received from patients, family members, facilities, and providers to assess needs and assign visits to team members as needed.
  • Assess and identify patient/caregiver problems, establish patient/caregiver care goals, and implement interventions for all agency programs.
  • Work with providers to establish and implement a plan of care for the patient.
  • Communicate effectively with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals and community service organizations.
  • Provide and respond to calls in a timely and professional manner for homecare, LTC, Kids Path, and other agency programs.
Requirements:
  • Current licensure as an RN in North Carolina or graduation from an accredited School of Nursing.
  • One year of nursing experience in triage or case management.
  • One year of hospice experience preferred.
  • Two years of experience in a triage or case management position preferred.
  • Understanding and commitment to the hospice philosophy of care.
  • Excellent assessment and clinical nursing skills, as well as effective interpersonal and communication skills.
  • Computer competency.
Additional Requirements:
  • Valid state-issued driver's license.
  • Automobile liability insurance at limits required by the agency.
  • Own transportation.
