Job SummaryThe Private Duty Nurse LPN will provide one-on-one care to patients in their homes, assisting with daily living activities and administering medications as prescribed. This is a rewarding opportunity to make a positive impact on the lives of our patients and their families.
Responsibilities- Implement and manage patient care plans in collaboration with physicians and other healthcare professionals
- Administer medications, treatments, and therapies as prescribed
- Conduct patient assessments and coordinate care with other healthcare providers
- Provide education and training to patients and their families on health promotion and disease management
- Document patient care and progress in a timely and accurate manner
- Current LPN licensure in the state of practice
- At least 6 months of hands-on nursing experience
- Current CPR certification
- Ability to work independently and as part of a team
