About the Job

Providence Humboldt Home Health is seeking a skilled and experienced Registered Nurse (RN) to join our team as a Home Health RN Specialist in Humboldt County, CA. As a key member of our healthcare team, you will provide high-quality patient care and support to individuals in their homes, promoting health, wellness, and independence.

Job Summary

We are offering a rewarding opportunity for a Home Health RN with at least 1 year of experience in skilled home health care, preferably with Oasis experience. You will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.

Responsibilities

  • Provide direct patient care, including assessments, interventions, and education to patients and their families.
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team.
  • Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
  • Conduct thorough initial assessments, on-going evaluations, and periodic reassessments to identify changing patient needs.
  • Maintain accurate and detailed records of patient interactions, interventions, and progress.
  • Promote patient safety, confidentiality, and dignity at all times.
  • Participate in regular team meetings and communication to ensure seamless coordination of care.
  • Ensure compliance with regulatory requirements, policies, and procedures.

Requirements

  • 1 year of experience in skilled home health care, preferably with Oasis experience.
  • Certification in Basic Life Support (BCLS).
  • Ability to maintain a valid driver's license and have reliable transportation.
  • Excellent communication, interpersonal, and organizational skills.
  • Able to work independently and as part of a team.
